Law on the Protection of Personal Data

DATA SUBJECT’S EXPLICIT CONSENT DECLARATION FORM

Within the scope of Law No. 6698 on the Protection of Personal Data, I hereby expressly give my permission and consent to the collection, recording, processing, storage, use, and transfer/sharing of my personal data by BAB İstanbul and its Website for the same purposes with third parties.

In accordance with Law No. 6698 on the Protection of Personal Data and related legislation, I confirm that I have received and read the information notice regarding the processing of my personal and sensitive personal data by the BAB İstanbul Website. I hereby give my explicit consent to the processing of all types of personal data, including data that identifies or could identify me, such as photographs and images, sensitive personal data provided verbally, in writing and/or electronically to the BAB İstanbul Website, and to the transfer and/or sharing of such data with third parties or through written or visual media as specified in the paragraph above. I also consent to be informed and contacted, and to the collection, use, transfer, and processing of my data in order to fulfill obligations arising from our relationship or required by administrative or judicial authorities.

I also acknowledge, declare, and undertake that I consent to the processing of my personal and sensitive personal data listed in detail below:

• Data that clearly belongs to an identified or identifiable real person, processed partially or fully automatically or non-automatically as part of a data recording system, and includes identity information such as full name, Turkish ID number, nationality, mother’s name, father’s name, place of birth, date of birth, gender; documents such as driver’s license, ID card, and passport, as well as information like tax number, social security number, signature, license plate, etc.
• Data that clearly belongs to an identified or identifiable real person, processed partially or fully automatically or non-automatically as part of a data recording system, and includes contact information such as phone number, address, e-mail address, fax number, IP address, etc.
• Data that clearly belongs to an identified or identifiable real person, processed partially or fully automatically or non-automatically as part of a data recording system, and includes personal data obtained during physical entry to or presence within a physical space, such as camera recordings, fingerprint records, and records taken at security checkpoints.
• Data that clearly belongs to an identified or identifiable real person and includes photographs and video recordings (excluding those under Physical Space Security Information) and data included in copies of documents containing personal data.
• Data that clearly belongs to an identified or identifiable real person, processed partially or fully automatically or non-automatically as part of a data recording system, and includes data defined in Article 6 of the Law on the Protection of Personal Data (e.g., health data including blood type, biometric data, etc.)
• Data that clearly belongs to an identified or identifiable real person, processed partially or fully automatically or non-automatically as part of a data recording system, and includes personal data related to any requests or complaints submitted to the BAB İstanbul Website.

I hereby confirm that I have read, understood, and accepted the BAB İstanbul Website’s Information Notice on the Protection and Processing of Personal Data and my rights under it.
